The Albert Journey Story
The Albert Journey began with the personal experience of the Melchior family. Albert Melchior, father of Matthew and Randy Melchior, lived with Alzheimerโs disease. His family walked alongside him through both the meaningful and difficult moments until his passing in 2010.

That experience gave the family a deep understanding of how dementia affects not only the person living with it, but also everyone travelling the journey beside them. It became the inspiration for Primacareโs long-standing commitment to improving dementia care.
From a Project to a Continuing Journey
Primacareโs approach has evolved through more than a decade of learning, collaboration and continuous improvement. The journey began with RegencyCareโs Living Tapestry and continued through the Albert Project, developed with partners including Baycrest, Western University and Crosswater Digital.

It later incorporated learning from Meaningful Care Matters, the Butterfly approach, DementiAbility and other person-centred dementia-care strategies. Over time, the Albert Project became The Albert Journey because dementia care has no fixed endpoint. It requires continued listening, learning and improvement as the needs of residents, families and team members evolve. We are grateful to the residents, families, team members, educators, researchers and partners who have travelled this journey with us. Our purpose remains the same: to provide dignified, compassionate and life-enriching dementia care. People focused = quality care.
A Thoughtful Approach
to Dementia Care
Albertโs Journey is Primacareโs approach to supporting residents living with dementia by focusing on the individual, not just the condition. By understanding each residentโs history, preferences, and needs, the program aims to reduce behavioural and psychological symptoms of dementia (BPSD) while creating more meaningful daily experiences.
Care is guided by a non-pharmacological-first approach, with a strong emphasis on engagement, consistency, and ongoing quality improvement.

Albertโs Journey continues to evolve, with a focus on expanding therapeutic options and strengthening staff expertise to better support residents living with dementia.

Residents benefit from a wide range of non-pharmacological therapies designed to support engagement, comfort, and overall well-being, including:
- Music therapy
- Snoezelen multi-sensory environments
- Validation therapy
- Pet animal therapy
- Art and horticulture therapy
- Aromatherapy and massage
- Doll therapy
- Laughter therapy
These therapies are integrated into daily care to create meaningful and supportive experiences.
Advanced Staff Training
Our teams are equipped with specialized training to ensure a consistent and confident approach to dementia care.
This includes:
- Gentle Persuasive Approaches (GPA)
- PIECES
- MoCA and MMSE
- U-First
- DementiAbility
Our goal is for a minimum of 80% of team members to receive advanced dementia training, supporting high-quality care across all homes.
